How have you gone about setting long-term and short-term goals? Have you done this alone or with family/peers/teachers/etc At home or in school?
Before we go into disability support in the college setting, let's take a moment and identify what is going on for you right now. Having a good understanding of your goals and supports you have used to reach them can be significant for your post-secondary successes
In Kindergarten through Transition, your goals are highly structured by the school. The supports you receive here are documented in an Individualized Education Plan (IEP).

Let's look a little more at how disability support is maintained prior to leaving high school or transition programs. We will also learn how your IEP can help support you reach your college and career-ready goals.

Now is your opportunity to reflect on your post-school goals and how your see your disabilities. Before you leave the transition program you will work with your case manager to develop a Summary of Performance (SOP). Having a copy of your current IEP may be helpful as your complete this activity.
